Welcome to Lifetime Family Wellness! We want you to feel part of our family from the moment you walk in the door. We understand that visiting a new doctor’s office can be nerve-wracking and we want to do everything we can to alleviate any concerns you may have before you even arrive.

Forms and Applications

Please take a moment to download and print our Application for Care. Fill it out as completely as possible and bring it to your new patient appointment. If you are unable to complete the application, please arrive about 10 minutes early to your appointment and we will take care of it then.
